Description:
Mount Sinai Morningside is seeking a dedicated Virtual Support Assistant – Palliative Care at Home to support clinical teams in providing care coordination services for patients receiving palliative care in a home-based setting.
The selected candidate will assist with patient outreach, scheduling, consent coordination, order follow-ups, and administrative support while working closely with healthcare professionals. This role requires strong organizational skills, effective communication, and the ability to manage patient care activities in a healthcare environment.
Key Responsibilities:
• Conduct patient outreach, provide program education, obtain consent, and coordinate enrollment scheduling.
• Support interdisciplinary care teams in addressing patient and family care needs.
• Assist with admission processes and ongoing home-based patient care operations.
• Follow up on provider orders and coordinate with clinical teams and vendors for successful completion.
• Provide administrative support for tracking enrollments, appointments, discharge orders, and care activities.
• Communicate care plans and patient assessments with frontline healthcare teams.
• Participate in regular team meetings and care coordination discussions.
• Maintain accurate documentation as required.
Requirements:
• High School Diploma or equivalent.
• 1+ years of healthcare experience.
• Experience with online ordering systems and data entry.
• Ability to work day and evening shifts, rotating holidays, and on-call schedules.
• Strong organizational and communication skills.
• Preferred experience:
• Associate Degree in Healthcare or a related field
• Experience working with EMR systems
• Strong telephonic communication and care coordination skills
• Spanish speaking proficiency is an advantage
