About the role:
Charlie Health connects people to personalized, virtual behavioral healthcare. As a Discharge Planner, you will coordinate care transitions and manage communication between our clients and referring organizations (hospitals, outpatient clinics, schools, government agencies, etc.). Your work helps ensure clients leave treatment with a clear, clinically appropriate aftercare plan and referral partners receive timely, professional treatment updates.
Key responsibilities:
• Provide consistent, high-quality treatment updates and professional communication to referring providers.
• Build and maintain trusting relationships with referral sources to enable seamless collaboration.
• Develop comprehensive discharge plans that promote successful aftercare for clients and families.
• Identify appropriate post-discharge providers using a nationwide provider database and make referrals via each provider’s preferred channels.
• Follow up with clients and receiving providers to confirm placement and continuity of care.
• Ensure receiving providers have all necessary clinical materials and coordinate any additional information requests.
• Document case contacts and discharge-planning activity in progress notes and communicate updates to therapists, case managers, and the treatment team.
• Collaborate closely with internal teams (clinical, admissions, Clinical Outreach and Partnerships) to support referrals and discharge tasks.
• Adhere to company policies, procedures, and performance metrics.
Requirements:
• Bachelor’s degree in a relevant field (health sciences, communications, psychology, social work, or similar).
• Minimum 2 years of relevant experience (case management, discharge planning, referral relations, admissions, outreach, or related patient-facing roles preferred).
• Strong interpersonal, relationship-building, and active-listening skills.
• Results- and metrics-driven with experience meeting concrete targets.
• Excellent written and verbal communication skills.
• Highly organized with strong attention to detail.
• Authorized to work in the United States and native or bilingual English proficiency.
• Ability to thrive in a fast-paced environment and learn quickly.
• Proficiency with Salesforce and Google Suite/MS Office.

• The expected base salary range at hire is $52,500 to $60,000 per year; final base pay is individualized based on location and experience and may be supplemented by incentive pay, bonuses, and other benefits.
