Responsibilities
The Virtually Integrated CareSM (VICSM) is a transformational model of care delivery offered by CommonSpirit Health that provides 24/7 virtual access to highly experienced RNs for patients and the bedside care team. With the VICSM program, the patient experience is enhanced and the interprofessional care providers are supported with time-consuming care activities such as charting, discharges, admissions, care coordination, and patient education (just to provide a few examples) leading to a reduction in nurse turnover, enhanced operational efficiency and unparalleled quality in patient care delivery. This is realized through a care delivery model that seamlessly integrates the virtual nurse as part of the interprofessional team. The virtually integrated careSM nurse is integral in facilitating the care delivery model.
- Collaborates virtually with physicians, nurses and other health care team members in the provision of clinical patient care, including virtual assessment of the medical, physical and psychological status of patients and responding to routine, urgent and emergent patient issues.
- Proactively oversees the patient care plan to implement preventive measures that improve patient safety and the quality of care delivered via telemedicine technology and surveillance of physiological trends; drives risk reduction through participation in root cause analysis (RCA’s), quality surveillance as well as the compilation and interpretation of large data sets of patient information.
- Effectively coordinates communication with members of the bedside interdisciplinary team.
- Supervises the comprehensive care of patients through coaching and mentoring bedside nurses and other care team members.
- Collaborates with unit leadership and provides oversight of unit quality programs and metrics; ensures evidencebased practices are being followed by team members to provide the highest quality care and safety for patients.
Qualifications
Required:
- Graduate of an RN nursing program and active RN license
- One year leadership experience required (e.g. Charge RN or related experience).
- Basic Life Support (BLS)
- Advanced Cardiovascular Life Support (ACLS) within 5 months of hire.
Preferred
- BSN, Bachelors of Science in Nursing
- Minimum 3-5 years in MedSurg/PCU/ICU experience within the last 2 years.
