Key Responsibilities:

• Conduct telephonic pre-visit check-ins with patients to collect and document medical information

• Perform daily patient calls to gather visit-related details and complete registration processes

• Review medication history, reason for visits, and identify gaps in patient care

• Coordinate with healthcare providers and clinical staff for smooth patient flow

• Maintain accurate and timely documentation using EMR/EHR systems

Requirements:
