knownwell is seeking a Patient Experience Coordinator to serve as the first point of contact for patients seeking support and information about healthcare services. This fully remote role requires excellent communication skills, empathy, problem-solving abilities, and the ability to manage patient interactions while ensuring a seamless and positive experience.
Key Responsibilities:
• Manage high-volume inbound and outbound patient calls while providing professional and compassionate support.
• Schedule, reschedule, and update patient appointments across multiple healthcare services.
• Conduct proactive patient outreach to coordinate provider-recommended care, follow-up visits, referrals, and specialty services.
• Review patient requests, provider orders, and referral details to ensure timely scheduling and appropriate next steps.
• Accurately document patient interactions, outreach attempts, and scheduling outcomes within electronic health record (EHR) systems.
• Leave detailed voicemail messages and track follow-up activities according to established procedures.
• Assist new patients with onboarding activities, including app access and appointment preparation.
• Monitor patient service queues and ensure requests are resolved within required service timelines.
• Escalate clinical questions, urgent concerns, and complex cases to the appropriate care teams.
• Maintain service quality while meeting productivity expectations related to calls, documentation, and patient follow-ups.
• Collaborate with clinical teams, operations staff, and leadership to improve the overall patient experience.
• Follow standardized workflows and documentation processes to maintain compliance and consistency.
Requirements:
• High school diploma or equivalent required.
• 5+ years of experience in medical scheduling, patient access, or healthcare call center environments.
• Strong verbal and written communication skills with the ability to clearly explain information to patients.
• Excellent multitasking and time management skills in a fast-paced healthcare environment.
• Strong attention to detail and ability to accurately document information in electronic systems.
• Experience working with electronic health record (EHR) platforms; Athena experience preferred.
• Ability to navigate multiple systems while communicating with patients.
• Strong problem-solving skills with the ability to remain calm under pressure.
• Patient-first mindset with a commitment to delivering compassionate and high-quality service.
• Experience in patient access, care coordination, referral management, healthcare scheduling, or telehealth is preferred.
• Bilingual language skills are an advantage.
