Patient Engagement Specialist — Remote (United States)
Evergreen Nephrology partners with nephrologists to deliver value-based, person-centered kidney care. We focus on improving patient outcomes and quality of life by slowing disease progression, enabling care at home, and accelerating transplants. Our interdisciplinary teams support clinicians with clinical resources, analytics, tools, and patient services that put patients first.
As a Patient Engagement Specialist you will be on the front lines of patient outreach and support — helping new patients engage with Evergreen and connecting them to their local care teams. You’ll work closely with market operations and clinical staff to drive patient activation, support onboarding, and help patients follow care plans that improve outcomes. This role also contributes to building contact center processes and partnership with senior leadership as the team grows.
Role Responsibilities:
• Manage a high volume of inbound and outbound calls focused on new patient onboarding
• Build rapport and trust with patients and clinical team members through exceptional omnichannel service
• Actively listen to patient needs, provide personalized support, and assist with next steps (including scheduling visits)
• Coordinate with interdisciplinary care team members — nephrologists, nurse care managers, APPs, social workers, dietitians, and practice staff
• Handle patient and practice administrative requests such as onboarding outreach, connection to local teams, scheduling help, and intake of complaints
• Help develop and document new contact-center workflows, processes, and knowledge resources
• Execute engagement initiatives tied to Clinical Excellence, Clinical Quality, and Patient Engagement goals
• Perform other duties consistent with the role as assigned
Schedule & Remote Requirements:
• This fully remote role requires availability for one of the following Central Time shifts: 10:30 AM–7:00 PM CT or 7:30 AM–4:00 PM CT
• Home internet must support a minimum 25 Mbps download and 10 Mbps upload (hardwired Cable, Fiber, or DSL preferred)
• Evergreen will provide required telephony applications and equipment
• Employees must work from a dedicated, interruption-free space to protect patient PHI / HIPAA information
Required Qualifications:
• High School diploma or GED required; Bachelor’s degree preferred
• 3+ years of direct patient support engagement or customer service experience in healthcare (startup experience a plus)
• Demonstrated comfort in conversational, persuasive outreach — able to navigate introductory conversations with new patients about Evergreen’s care model
• Experience meeting workload metrics and collaborating in a team-based service environment
• Strong verbal and written communication skills; conversational intelligence and genuine curiosity when engaging patients
• Intermediate Microsoft Office skills (Outlook, Teams)
• Experience with Medicare and/or the kidney care population is a plus
• Bilingual Spanish/English fluency is a plus
Compensation:
• Hourly pay range: $19.00 to $25.00 (exact pay depends on experience, education, and role factors)
• This position is also eligible for an annual bonus
