Description:
Diverge Health is seeking a detail-oriented and mission-driven Medical Records Coordinator to support its healthcare operations focused on improving access, outcomes, and care coordination for Medicaid patients. The organization partners with primary care providers to enhance patient engagement and close gaps in care through structured clinical support and data management.
In this role, you will serve as a key link between healthcare providers and internal clinical teams by ensuring accurate collection, organization, and transfer of patient medical records. You will work within electronic medical record (EMR) systems to support timely and accurate clinical documentation that directly contributes to patient care quality and operational efficiency.
Key Responsibilities:
• Obtain and request medical records from hospitals, clinics, and healthcare providers
• Organize complete patient medical histories including labs, notes, and specialist reports
• Upload, attach, and maintain accurate documentation within EMR systems
• Transcribe and transfer clinical data into internal healthcare databases
• Follow up with external providers to resolve missing or incomplete documentation
• Ensure patient records are complete, accurate, and accessible for clinical teams
• Maintain compliance with HIPAA and data privacy regulations
• Collaborate with internal teams to support care coordination workflows
Requirements:
• Experience working with EMR platforms and healthcare record systems
• Strong understanding of medical terminology and clinical documentation
• Experience interacting with healthcare providers and administrative staff
• Familiarity with CRM or database systems such as Salesforce
• High attention to detail with strong organizational skills
• Ability to manage high-volume documentation in a fast-paced environment
• Strong understanding of:
