Description:

Innovative Integrated Health (IIH) is seeking a detail-oriented Health Records Clerk to support its healthcare team in maintaining accurate and complete participant health records. This role plays an important part in ensuring medical documentation is received, organized, uploaded, and available for effective care coordination.

The successful candidate will work closely with clinical teams, scheduling departments, intake teams, and external healthcare providers to manage health records and support participant care. This position requires strong attention to detail, confidentiality, and the ability to manage multiple administrative tasks in a healthcare environment.

Key Responsibilities:

• Prepare medical charts for new participants and ensure required health history documents are available for review.

• Scan and upload participant documents into electronic health record systems.

• Maintain participant records by collecting medical information including test results, specialist consultations, procedure reports, and hospital summaries.

• Coordinate with interdisciplinary teams to support participant care planning.

• Monitor scheduled external appointments and update related records.

• Route medical notes and documentation to appropriate providers for review.

• Review and maintain scheduled orders while tracking follow-ups and updates.

• Respond to medical record requests and perform general clerical duties.

• Maintain compliance with healthcare policies, procedures, and regulatory requirements.

• Participate in staff meetings, training sessions, projects, and assigned committees.

Requirements:

• Minimum 1 year of experience in Health Records is required.

• Minimum 1 year of documented experience working with elderly or frail populations.

• High School Diploma or equivalent.

• Medical Assistant Certification is preferred.

• Medical Terminology Certificate is preferred.

• Current Driver’s License in the State of California.

• Strong computer skills with proficiency in Microsoft Office applications.

• Knowledge of electronic health records systems and office procedures.

• Excellent communication skills with strong attention to detail and accuracy.

• Ability to manage changing priorities and work independently.

• Strong knowledge of:

• Electronic health records management and medical documentation.

• Medical terminology, filing systems, and healthcare administrative procedures.

• Microsoft Word, Excel, Access, PowerPoint, and Outlook.
