Centene Corporation – Care Management Support Coordinator II
Centene is looking for a dedicated Care Management Support Coordinator II to assist in administrative care management activities and serve as a key point of contact for members, providers, and staff. This role helps ensure high-quality support and guidance throughout the care process, documenting member interactions in compliance with state and regulatory standards.
Key Responsibilities:
- Conduct outreach to members via phone to guide them through care plan next steps, connect them with community or health plan resources, and provide education to both members and providers
- Support members in accessing health plan and community resources to ensure optimal care
- Apply working knowledge of assigned health plan activities and resources
- Serve as first-line support for member and provider inquiries, including explaining care plan procedures and protocols
- Assist with member onboarding and administrative duties, including sending welcome letters, correspondence, and educational materials
- Document and maintain non-clinical member records in compliance with state and regulatory requirements
- Refer members to appropriate local benefits and services to address Social Determinants of Health (SDOH) needs
- Perform additional duties as assigned while adhering to company policies and standards
Requirements:
- High School Diploma or GED
- 1–2 years of related experience in healthcare or care management support
- Excellent communication and organizational skills
- Ability to work remotely with a flexible schedule (Monday–Friday, 8am–5pm EST or 9am–6pm EST)
