Description:
Charlie Health is seeking a dedicated Discharge Planner to support clients and referral sources by providing high-quality treatment updates and coordinating discharge plans. Our mission is to expand access to personalized, virtual behavioral healthcare for individuals with complex needs, including mental health conditions, substance use disorders, and eating disorders.
Key Responsibilities:
• Provide consistent, professional treatment updates to referral providers
• Develop and execute comprehensive discharge plans for clients completing treatment
• Identify appropriate aftercare resources and facilitate smooth transitions
• Maintain documentation of case management contacts, collaborating with therapists, case managers, and treatment teams
• Communicate effectively with referral sources and internal stakeholders to ensure seamless client support
• Adhere to organizational policies and performance metrics
Requirements:
• Strong interpersonal, relationship-building, and listening skills
• Metrics- and results-oriented with experience working against concrete targets
• Excellent written and verbal communication abilities
• Highly organized with attention to detail
• Authorized to work in the United States with native or bilingual English proficiency
• Ability to work in a fast-paced environment and learn quickly
• Proficient in Salesforce, Google Suite, and MS Office
Preferred Qualifications:
